CHILD CARE AGREEMENT
THIS AGREEMENT is entered into as of this ____day of _____2010 in Location, by and between (Home Sweet Home Group Family Daycare) (hereinafter referred to as "Provider") and _________________________(hereinafter referred to as "Parent/Guardian").
This Agreement contains the terms agreed upon between Provider and Parent/Guardian for the care of:

1. Security/Holding Deposit. Parent/Guardian agrees to pay security/holding deposit of $ 200.00
At the termination of the agreement for care, all required fees will be deducted from the security/holding deposit. Any remaining amount will be credited towards the last two weeks of care at Home Sweet Home Group Family Daycare.

2. Weekly Rate. The weekly rate will be $________ and is due and payable each Friday by 5:30pm. If Child is absent from care the normal weekly rate will be due and payable.
3. Days and Hours. The parties to this agreement have agreed to the following schedule of care.

4. Late Fees. Parent/Guardian agrees to pay a late fee of $1.00 per minute or portion thereof that Child remains in care after the hours listed in
Section 3.
If the weekly rate is not paid by 5:30pm each Monday, Parent/Guardian agrees to pay a late fee in the amount of $10.00 per day until the account is current.
All late fees are due and payable immediately.
5. Provider Vacation. Provider will make adequate and necessary provision when on vacation.
6. Term. The Agreement terminates on June 26, 2010. Failure to comply with the terms set forth in this Agreement may, at Provider's discretion,
result in immediate termination of Child's enrollment and forfeiture of the security deposit.
